Have you tried terbinafine cream? I hunted down some effectiveness studies because I had a persistent infection, and that was shown to be the most effective. Sure enough, it cured it.

Of course you also need to clean your floors, especially your tub or shower floor, and wear clean, well-ventilated shoes if possible, or at least have other pairs you can rotate so they can always stay dry inside. If they’re already nasty you may want to take out the insoles and thoroughly disinfect them.
